Adult Care Act - Referral
Please choose one of the three forms below to apply for a Care Act Advocate if the following two apply:
- Have a ‘substantial’ difficulty
- Do not have an appropriate person to help
and are going through one of the following processes:
- Carer’s Assessment
- Care Review
- Care Planning
- S42 Safeguarding
- SAR (Safeguarding Adults Review) they are eligible for Care Act Advocacy
To access Care Act Advocacy at Embrace you need to be a resident of Wigan Borough. All local authorities will have an appointed independent advocacy service in their area.

Please book a telephone appointment if you are not a professional and would like to refer someone for support.
